I am a citizen of the United States eligible to hold such office under the constitution and laws of this state. I have not been determined by a final judgment of a court exercising probate jurisdiction to be totally mentally incapacitated or partially mentally incapacitated without the right to vote. I am aware of the nepotism law, Chapter 573, Government Code. I am aware that I must disclose any prior felony conviction, and if so convicted, must provide proof that I have been pardoned or otherwise released from the resulting disabilities of any such final felony conviction. I am aware that knowingly providing false information on the application regarding my possible felony conviction status constitutes a Class B misdemeanor. I further swear that the foregoing statements included in my application are in all things true and correct.”

Democratic Party Oath of Affiliation
In accordance with Texas Election Code §161.006 – 008; "I swear that I have not voted in a primary election or participated in a convention of another party during this election year. I hereby affiliate myself with the ____DEMOCRATIC____ Party."
FURTHERMORE, In accordance with Texas Election Code §161.005; "I swear that I am a qualified voter in my election precinct and I am not a candidate for nomination or election to, or holder of, an elective office of the federal, state, or county government."
AND, I also understand that according to Texas Election Code §171.023.(c); precinct boundaries may be subject to change (effective Feb 1st) prior to the primary election, and this may affect my eligibility to remain in the office for which I am applying.
